Chaparral Medical Group in partnership with Health Care Avenues is working with San Bernardino County to provide the COVID-19 vaccine to our community.

Currently, offering Pfizer Bivalent Booster (age 12+) and Moderna Bivalent Booster (age 18+) options only.

​We are now using the new CA site called MyTurn for appointment registrations.  
• Go to https://myturn.ca.gov/
• Click on "Find an appointment"
• Input your info and select "San Bernardino" county
• Enter zip-code "91730" and select "Rancho Cucamonga"
• Scroll down and find "Chaparral Medical Group" for your vaccine brand and click on "Schedule appointment"
• Choose your desired date/time and complete registration process
